Essential Blue Basic is the least expensive policy we offer. Keep in mind, this limited benefit plan does not have benefits for most outpatient care (like doctor's office visits, lab and X-ray and prescription drugs).
Benefit Summary
Essential Blue Basic is a limited benefit plan, meaning this plan is NOT designed to cover all medical expenses. Services Not Covered under Essential Blue Plus include; Doctor office visits, prescription drugs, most outpatient services, Chiropractic Care Services, Mental and nervous conditions, Durable Medical Equipment, Orthotic Devices, Outpatient Occupational, Physical & Speech Therapy's, Psychiatric Services and Growth Hormone Therapy. This plan is often referred to as a "hospital only" plan.
Deductible Choices:
PRIMARY BENEFITS
- After the deductible is met: You pay 20% of the cost for most covered medical expenses. Coinsurance liability is limited to $2,000 per person
- Annual Maximum Benefit: $1,250,000 per insured
- Hospital Services: You pay 20% of the allowed amount for covered services after meeting deductible (Limited to inpatient care, outpatient surgery and preadmission testing)
- Pregnancy Services:You pay 20% (Seperate $5,000 deductible applies)
- Doctor office visits: Not Covered
- Prescriptions: Not Covered
- Preventive Care & Wellness: Not Covered
