Dental Blue^® Preferred Provider Organization (PPO) is an arrangement under which certain dental care providers are contracted with Blue Cross of Idaho to form a network that offers you flexible, affordable dental coverage.

We understand how important it is for you and your family to feel confident about your choice of dentists. With Dental Blue PPO, you are free to choose any dental care provider inside or outside the PPO network, although you maximize your benefits when you use providers that have a Dental PPO contract with Blue Cross of Idaho.

High-Quality Care at a Reasonable Cost
In-network Dental Blue PPO providers agree to write-off any portion of their fees that exceed the maximum allowance for services you receive. So, even if the care you receive costs more than the allowable amount, you will be responsible only for the deductible, coinsurance, copayment and any noncovered amounts. If you choose to see an out-of-network provider, you will be responsible for any deductible, coinsurance, copayment, noncovered amounts, in addition to any amounts that exceed our maximum allowance. By choosing an in-network provider, you maximize your dental care benefits.

Annual Maximum Benefit Amount
Dental Blue PPO offers you the flexibility of two different annual maximum benefit amounts. You may choose a benefit amount of $1,000 or $1,500, depending upon the needs of you and your family. The annual maximum benefit you choose is per insured per calendar year.

Preventive Dental Services

In-Network Services: Dental Blue PPO pays 100% of the maximum allowance for covered preventive care services after a $20 copayment. Preventive services include annual examinations and routine cleanings. The deductible does not apply to in-network preventive care covered services.

Out-of-Network Services: Dental Blue PPO pays 70% of the maximum allowance after the deductible has been satisfied for out-of-network preventive care services.

Basic Dental Services

In-Network Services: After the deductible and six-month waiting period have been satisfied, Dental Blue PPO pays 80% of the maximum allowance for in-network basic care services.

Out-of-Network Services: Dental Blue PPO pays 60% of the maximum allowance after the deductible has been satisfied for out-of-network basic care services.

Major Dental Services

In-Network Services: Once the deductible and 12-month waiting period have been satisfied, benefits for root canals, inlays, crowns, and other major dental services received from an in-network provider, Dental Blue PPO will pay 50% of the maximum allowance.

Out-of-Network Services: Dental Blue PPO will pay 40% of maximum allowance after the deductible and a 12-month waiting period for out-of-network major care services.

Dental Services Outside Idaho
Blue Cross of Idaho pays out-of-network benefits for services provided outside Idaho.

Application Requirements:

• You must be at least 18 years of age and no older then 64 years of age.
• You cannot currently have Blue Cross of Idaho dental insurance.
• You must be an Idaho resident.
